Hematoma formation under the skin flap is perhaps the most feared complication of facelift surgery. Although its risk is less after a mini facelift, it is nonetheless an important issue to consider. The cause of this complication is blood collection from a bleeding vessel, especially in males and patients with hypertension. Minor clots can be drained in the office under local anesthesia, while major hematomas require drainage in the operating room. This specific complication can be significantly lowered by suspending blood thinners before the operation and strictly controlling blood pressure.
