Eyelid retraction is a complication of lower eyelid surgery. It occurs when too much skin is removed, or eyelid laxity is not addressed during the operation. The best way treat eyelid retraction is by avoiding it in the first place. Nevertheless, when it does happen, the surgeon has a number of options to correct the problem. The first option is to recover lost skin by harvesting skin grafts from an area behind the ear (postauricular grafts). Skin grafts will be visible, though, and may not provide the best aesthetic outcome. For this reason, many surgeons prefer to fix eyelid retraction with an operation called midface life with bone fixation. A midface lift is an elegant way to treat eyelid retraction that consists of recruiting skin from the cheeks to restore eyelid shape without the use of a skin graft.
